Whenever possible, we aim to collect personal information directly from the individual it relates to, or their parent/caregiver or representative.   We may collect your personal information by various means including:

  • Online intake forms via secure link 

  • Email

Use of  Your Information

We may collect your personal information to provide Occupational Therapy including: assessment, notes, reports, correspondence or other therapy purposes

 

We will also disclose your information if required by law to do so or in circumstances permitted by the Privacy Act – for example, where we have reasonable grounds to suspect that unlawful activity, or misconduct of a serious nature, that relates to my functions or activities has been, is being or may be engaged in, and in response to a subpoena, discovery request or a court order.

Our policy with regard to sensitive information obtained from minors:

Peaceful Playful & present support children and those who love them. As per relevant Federal and State legislation, health service provider is required to keep records for 7 years or, in the case of a child, until the child turns 25.

Security

We are committed to securely storing and handling your sensitive information and take reasonable physical, technical and administrative safeguards to protect your personal information from misuse, interference, loss, and unauthorised access, modification and disclosure. If we no longer need, or are no longer required under Australian law to store your personal information, we will de-identify and/or destroy the information.

We manage risks to your personal information by holding electronic data in a secure Client Record Management system. With password protection which is only accessible to staff od Peaceful Playful & Present. Other electronic data is stored on password and finger print protected computer and hard drive.

Access to Information

You can contact us to access, correct or update your personal information at any time. Unless we are subject to a confidentiality obligation or some other restriction on giving access to the information which permits us to refuse you access under the Privacy Act, and we believe there is a valid reason for doing so, we will endeavour to make your information available you.
